We appreciate all the years with Rogue Valley Physicians and wish them the very best.

As patient letters stated, the office at 800 E Main Street will be permanently closed to G&R patients as of 4/2/2026 (a new business will be in this location). Patient visits have been concluded as of 3/17/2026.

You have the choice of medical providers including one of the other clinics of Rogue Valley Physicians, you can contact one of the following offices to request to establish care:

A Medical Records release is required. If you’d like to have your records transferred to a new provider office, please print the Medical Records Release form and fax it to 541-608-7689 or mail it to 2900 Doctors Park Drive, Ste 200, Medford OR 97504.
